Authorization for direct deposit

Follow the easy steps below. Be sure to review the checklist below before submitting your request.  

Steps     

  1. Complete Authorization for Direct Deposit form
  2. Send the completed Form to: Fidelity & Guaranty Life Insurance Service Center, P.O. Box 81497, Lincoln, NE 68501-1497.
    • For overnight mailing send to: Fidelity & Guaranty Life Insurance Service Center, 777 Research Drive, Lincoln, NE 68521.
  3. Your request can also be faxed to: Fidelity & Guaranty Life Insurance Service Center, at the fax numbers listed alongside. Please Note: allow 3 business days for faxed documents to enter our processing system.
  4. Please allow up to 30 days for the direct withdrawal to be set up. You will likely need to remit one more premium via your old method or bank account.

Checklist

Checking Accounts: Make sure to include a voided check with the form. Please do not send a deposit slip as they usually list a different routing number.

Savings Accounts: Make sure to include a deposit slip or other document listing both the account number and routing number from your bank.

Sign the form: Please make sure to sign the form.
